The Canadian Food Inspection Agency (CFIA) has designated a primary control zone for a premises in the Guelph area with an outbreak of highly pathogenic avian influenza.
Therefore, a permit is required to ship poultry samples or carcasses to AHL.
A CFIA general control permit is available at https://inspection.canada.ca/en/about-cfia/find-form/cfia-acia-5752 [1].
A copy of the prefilled permit with AHL details is available here [2].
Please complete the permit, request a 3 month movement period, and forward to the CFIA email listed for approval and signature.
Once you receive your approved permit, please attach to each AHL submission – either hard copy with your courier package, or electronically to specroom@uoguelph.ca [3].
This must continue until the control zone is revoked by CFIA.
