University of Guelph COVID-19 Screening Form
To help reduce the spread of COVID-19 and protect our community members, the University of Guelph requires all visitors to complete the U of G COVID-19 Screening Form every day before coming to any U of G facility, location or campus (University Locations). If the form indicates the individual has symptoms of COVID-19 or should isolate, the individual is not permitted to attend any University Location.
You can access the Screening Form at https://uoguel.ph/COVID19screening or through the Safe Gryphon app available from the App Store for iPhones or Google Play for Android.
The Screening Form includes a link to the Ontario government’s COVID Self-Assessment Tool (Assessment Tool).
Anyone who attends the University Locations without completing the Screening Form or who falsely reports the results of the Assessment Tool or does not comply with the University’s COVID-19 safety practices will not be permitted at any University Locations.
Who Should Complete the Screening Form?
The Screening Form should be completed by individuals who plan to come to the University Locations every day prior to their visit.
The Screening Form should not be completed by an organization’s representative, such as supervisors, on behalf of their team members.
On behalf of your organization, please forward this message to all individuals who plan to attend work sites at any University of Guelph Locations including any personnel from your organization’s subcontractors.
What Information Will Be Collected and How Will It Be Used?
To complete the Screening Form, the individual will be required to provide personal information including their name, email address and phone number. This personal information will be collected by the University pursuant to the University of Guelph Act 1964 and consistent with Ontario’s Freedom of Information and Protection of Privacy Act. Personal information will be retained for a period of five years or as otherwise required by law.
If an outbreak of COVID-19 at a U of G Location takes place and may affect the individual, this personal information may be disclosed to the Wellington-Dufferin-Guelph Public Health or other applicable Public Health Unit to assist with contact tracing efforts.
The information you provide in the Ontario government’s Assessment Tool is not collected by the University.
