New: A Research Ethics Online System – EthOS – is now available as of July 25, 2024.
EthOS [1]replaces U of G REB forms and supplements previously completed in Microsoft Word.
As of July 25, for any research involving human participants, please use EthOS [1] for all main (standard) REB applications (including Multi-jurisdictional and Request for Exemption applications) and Statement of Intent requests.
The legacy forms below may only be used under the following three scenarios:
- You recently submitted a protocol using the Microsoft Word REB application form, and it is under review by the Research Ethics Board.
- You have an existing active, approved protocol that was created using the Microsoft Word REB application form and need to amend or report ongoing activity on your project before September 23, 2024 (see note below).
- You began drafting an REB application using the Microsoft Word form, have not yet submitted it as of July 25, 2024, and prefer not to re-enter the content into EthOS. The last day to submit any applications using the Microsoft Word form is September 22 2024.
Note: After September 23, 2024, all previously approved, active pre-EthOS protocols will be uploaded to your EthOS project list and users can process amendments, renewals, event reports, and study closures in EthOS.

Application forms
Main REB Application Form
The former REB Application form is no longer available for download. As of July 25, 2024, all new applications are submitted through EthOS [1] although any applications completed using the former REB application will be accepted until September 22, 2024. If you require a copy of the former REB Application form, please contact reb@uoguelph.ca [2].
Statement of Intent for Release of Funds Prior to Ethics Approval
If your research involves human participants at any point, a valid REB number must be provided to Research Support Services or the Agri-Food Partnership office prior to account setup. If it is not feasible to obtain REB clearance at this stage, but you must access funds, please submit a Statement of Intent for Release of Funds.
The former Statement of Intent application form is no longer available for download. As of July 25, 2024, all new SOIs are submitted through EthOS [3]. If you require a copy of the former Statement of Intent form, please contact reb@uoguelph.ca [2].
Supplements
- Supplement I: Sona [4]
- Include ONLY IF you intend to use the SONA system (Psychology and Marketing & Consumer Studies).
- Supplement II: Secondary Use of Data [5]
- Include ONLY IF this project will involve the secondary use of data or human tissue samples.
- Supplement III: Cross Cultural Research [6]
- Include ONLY IF this project involves Indigenous peoples, communities or lands, or any culturally distinct identifiable group.
- Supplement IV: Biological Specimens [7]
- Include ONLY IF this project involves the collection of human biological samples.
- Supplement V: Clinical Trials [8]
- Include ONLY IF this project is a clinical trial. See Chapter 11 [9] of the TCPS2 for the definition of a clinical trial.
- Supplement VI: Course-Based Research [10]
- Include ONLY IF this involves research undertaken by students, for pedagogical purposes.
Amendment Form
Use the Amendment Form [11] if you are altering an approved submission that must be reviewed before September 23, 2024. As of September 23, 2024, amendments to all approved protocols will be processed in EthOS.
Annual Renewal Form
Use the Annual Renewal Form [12] if you wish to renew a submission for another year or close a submission before September 23, 2024. As of September 23, 2024, renewals and closures of all approved protocols will be processed in EthOS.
Event Report
Use the Event Report form [13] if something occurs during the course of your research project that should be communicated to the REB before September 23, 2024. As of September 23, 2024, event reports for all approved protocols will be processed in EthOS.
The form provides guidance on the types of information that must be reported, but feel free to use this form to report any occurrence you feel is important. Check with the Manager, Research Ethics if you are not sure about reporting.
