Hiring/Tap Access
- Hiring Form
- TD1 Forms - now submitted by student/staff through SharePoint
- Payroll Direct Deposit Form - now submitted by student/staff through the Employee Self-Service Portal
- Tap Access Form
- Submit completed, signed form and a single pdf with proof of completion of the required safety courses (Lab Safety, EHS Biosafety, WHMIS, EHS Worker Health and Safety Awareness, Principles of Belonging) to Lorraine Weir at ibtap@uoguelph.ca.
- Autoclave access: The IB department has two autoclaves available for use. All autoclave users must be trained by Shaylah Tuttle-Raycraft prior to gaining access to the autoclave rooms.
- Phytotron access: Training must be completed with Michael Mucci to gain access.
- Genomics access: Training must be completed with Scott Ryan to gain access.

Volunteer Forms
These forms must have the Faculty Member's signature and name printed on them and have the Chair's signature. Please forward completed form along with a single pdf of the required safety courses' proof of completion to Lorraine Weir to obtain the Chair's signature. Forms can not be signed until all the required safety courses have been completed and proof of completion has been submitted. Required safety courses include: Lab Safety, EHS Biosafety, WHMIS, EHS Worker Health and Safety Awareness, Principles of Belonging.

Electronic Repairs
