University of Guelph Notice of Offence

1. Print Name: the Campus Community Police Officer who issued the Notice of Offence 

2. Day of: the date the offence occurred as opposed to the date the ticket was served (see 5)

3. Breach: description of the alleged breach

4. Section: section of the Policy on Non-Academic Misconduct alleged to have been breached by the student

5. Date Served: the date the ticket is issued to the student -- in some cases, there is an investigation and the ticket is not issued until days after the occurrence of the breach
