OR-5 Signature Approval

The OR-5 Research Proposal Approval Form must be completed for all research proposals before submission to research sponsors, and must be signed by the Principal Investigator, the Departmental Chair or School Director, and the College Dean before submission to and authorization by the Office of Research. All signatures on the OR-5 Form signify agreement with Meaning of Signatures, and awareness and approval of the proposed research moving forward in the application/approval process.

The OR-5 form has been updated to accompany the launch of the Full Cost of Research Policy, effective May 1, 2024.  The updated OR-5 form below refers to the Full Cost of Research Policy, exceptions and exemptions. In addition, information buttons have been added to make the form more user-friendly. Please bookmark the following page for future submissions:

• Please download the form to complete the information, and then save and attach to the intended e-mail message since changes may not be saved if the form is opened and completed in an Internet browser.
• PREVIOUS OR-5 FORM VERSIONS: The following versions of the OR-5 form will be accepted until October 31, 2024. 
  • OR-5 in Adobe, electronic signatures using Digital ID
  • OR-5 in Adobe, electronic signature for Principal Investigator by inserting an image (e.g. JPEG file)
  • OR-5 in Adobe, without electronic signatures
